Doula + CHW Birth Village Care Coordination:

This care coordination follows a Community Health Worker (CHW) model focused on assessment, care planning, referral follow-up, and barrier reduction.

• In-person needs assessment (pregnancy + postpartum)
• Personalized Care Map (support plan + next steps)
• Resource navigation + referrals
• Follow-through check-ins to help reduce barriers and support completion of next steps
